Patient Services Portal – register and login for online health appointment scheduling and access to your medical records and more.

At Santa Rosa Orthopaedics you may schedule an appointment without being referred by another doctor. However, if your insurance plan requires a referral from your primary care provider, you will need to obtain this referral before arriving for your appointment.

Our offices are open from 8:00 am until 5:30 pm Monday through Friday. Patients can schedule appointments from 8:30- 5:00. Early notification of cancellations is appreciated so that your appointment may be filled with another patient and avoid delays. You may be charged for a missed appointment, untimely cancellation of an appointment, surgical cancellation fee for late cancellation and or failure to comply with surgical instruction/s.

Please bring all pertinent medical records with you from your primary care provider or other orthopaedic physician. These may include:

  • Insurance identification
  • X-rays and/or MRI scans
  • List of current Medications
  • Laboratory results

From Outside Physician

  • Operative reports
  • Arthroscopic photographs
  • Videos or pictures of prior operations

Calling Santa Rosa Orthopaedics

Dial 546-1922 and select option 1 to reach our central scheduling department. Any of our schedulers may assist you in scheduling an appointment with any of our physicians.

For your convenience, you may request to see a specific physician or you may choose to be referred to any of our outstanding physicians according to the specific specialty you require or simply by earliest availability. If you have a matter that requires immediate attention, you may be seen in most cases by the physician on call for the group that day.

Scheduling Appointments

If you wish to schedule an appointment with a specific physician, please select from the physician contact list.

For questions regarding medical referrals, procedures or prescriptions, please contact your Physicians’ Medical Assistant – do not call the Physician’s Medical Assistant for appointments, instead follow the appointment procedures above.

We look forward to meeting you at your scheduled appointment. Please arrive at least 30 minutes before your scheduled appointment time. Allow at least one and a half to two (1.5 to 2) hours for your first appointment.


At the time of your appointment please bring the following:

  • The two completed and printed forms Standard Patient Demographic Form & Standard Patient History Form
  • All current medical insurance cards
  • Referral form, if required by your health plan
  • Recent x-rays or MRI studies
  • A list of all current medications

If you are unable to keep your appointment, you must cancel it at least twenty-four (24) hours in advance. Failure to attend a scheduled appointment or notify the office with a 24 hours’ notice may result in a charge computable to your scheduled appointment.

We understand and respect the fact that in certain instances, reasonable and extenuating circumstances may prevent you from providing 24-hour notification of schedule changes. These cases will be weighed on an individual basis.

Santa Rosa Orthopaedics participates in most major insurance plans and will bill all insurance plans, whether participating, (contracted), or not contracted.

Please bring your insurance card at each visit to our medical or physical therapy offices.

In an effort to keep medical costs down we require all managed care co-payments be made prior to seeing your physician or therapist.  It is the responsibility of the patient to obtain the necessary referral prior to seeing a physician or therapist for the first time or for a new injury or complaint.

We do understand that insurance payments are sometimes delayed.  You will receive a statement after your insurance has notified us of any amount that is the responsibility of the patient/insured.

Private pay accounts are expected to pay at the time service is rendered.

NOTICE: Making a false or fraudulent workers compensation claim is a felony subject to up to 5 years in prison or a fine of up to $50,000 or double the value for the fraud, whichever is greater, or by both imprisonment and fine.

Workers’ Compensation Patient Registration Form

We try our best to handle your requests for prescription refills quickly. To order a refill, please call your pharmacy, and they will request your refill from our office.

Please do not contact the physician on call after normal business hours. Our physicians handle a high volume of emergency calls and we ask that our patients play an active role in monitoring the status of their own prescriptions, and plan ahead for weekends and holidays. In general we would appreciate it if you would allow at least 48 hours for prescription refills to be processed as in many cases your physician may be in surgery and not receive your request until the following day.
